BOTOX...There is Still Time!

What is Botox?
Botox Cosmetic is a purified protein (Botulinum Toxin Type A) produced by the bacterium Clostridium botulinum, that is injected directly into the tiny facial muscles that contract and cause wrinkles. Post-Baby Body Syndrome Revealed!
Pregnancy leads to changes in a woman’s body. After pregnancy many of the changes can be reversed either totally or partially. However, in some women the changes persist long after the post partum period despite good intentions and best efforts on their part.
Here is an example.
I had a neighbor years ago who had two children and after her pregnancies she noticed a pouch in her abdomen. She was very determined to improve and possibly resolve this problem. She hired a personal trainer and together they worked diligently for nine months. During that period of time she did 250 sit-ups in the morning and 250 sit-ups in the evening. The result of that was her muscles were strong but she still had the pouch in her abdomen.
What determines if the changes that occur will persist after post partum period?
The key here is what I call the shriveled balloon effect. It is basically the extent to which the skin and tissues have been stretched during the pregnancy that determines the tissues’ ability to snap back into their pre-pregnant condition.
For example: if you take a normal balloon and put a little puff of air in it and let the air out of the balloon, it will return essentially to its normal shape. If you take the same balloon and put air in it and keep expanding the balloon until it is about ready to pop and let the air out, the balloon is thinned and shriveled and it is not the same as what it was prior to blowing it up. Very similar thing happens in pregnancy to the skin of the breasts and abdomen.
For example: Let’s consider a woman that is about 5’ 2” and about 110 lbs. and gains 65 lbs in pregnancy. Often times the stretch that occurred during pregnancy are such that the body isn’t able to snap back into the shape it was before pregnancy. On the other hand, if the same 5’2”/110 lbs. woman gains only 25 lbs., the stretch is much less likely to persist and it is more likely she will have a relatively normal shape after the pregnancy.
What is the definition of the Post Baby Body Syndrome?
It is the changes that occur in a woman’s body after the postpartum period that persist despite exercise, diet, and nutrient treatment. These changes are often permanent. I discovered the syndrome after the following event occurred in the life of one of my patients.
We will call her Mary, though this is not her real name. She was scheduled for a Tummy Tuck. She is 43 years old, normal weight and great health. She runs several miles three times a week and does yoga twice a week. She has two teenage sons, and since their births she has had a persistent bulge and loose skin in the abdomen. She wanted to do something about it and finally decided to have surgery.
In the process of getting ready for her surgery she went to her Primary Care Physician to get routine blood work done. When her doctor saw her, Mary was told that she needed to lose weight and there would be no value of having a Tummy Tuck. Mary was very angry with her doctor, as it was obvious to her he did not fully understand her situation. Besides which she felt that the doctor was preaching to her. Much later, Mary shared with me the story of what had happened and how she felt that day.
What became obvious to me is that many of my patients who come in believe they are failures, because the effort they expended to get rid of the pouch in their abdomen, for example, were to no avail. I was shocked to discover that a medical professional was not even aware there can be persistent body changes in a woman after pregnancy and sometimes, no matter what they do in terms of exercise, diet or therapy… those changes persist.
What are the criteria for Post- Baby- Body Syndrome?
Often the skin is stretched and loose, and does not have the ability to snap back normally like skin with good tone does.
The fascia ( tissue holding the muscles) - particularly the muscles of the abdomen -has been stretched. No amount of exercise or sit-ups will help this skin to return to normal.
The pelvic area is often widened, looser and may have a slight tilt.
There are breast volume changes. Sometimes the breasts are larger but more often than not, there has been a shrinking resulting in a loss of breast volume. In addition, there can be a difference from one side to the other, with the breast being lopsided as a result.
Often times weight has been added to the hips and outer thighs. That, coupled with the widening of the pelvis, leads to a more matronly appearance. A matronly widening, if you would.
There can be changes in the floor of the pelvis resulting in dysfunction of urination and bladder control.
There are often some vaginal changes that occur as well.
In summary, women after pregnancy can generally be divided into two main groups. One group can return their body to normal or near normal with exercise and proper diet.
The other group of women find that despite all efforts - exercise, diet, etc. - the changes that have occurred to their body are permanent, persistent and do not respond to conservative modalities of treatment.

What You Should Ask Before Your Cosmetic Surgery Procedure!
How much experience do you have?
So far this year, our surgeons have completed nearly 1500 cosmetic surgery procedures.
This number includes breast augmentations, breast lifts/reductions, tummy tucks, liposuctions and rhinoplastys ("nose" jobs). We specialize in Breast Augmentation, Post-Baby Body changes, and after weight loss body re-contouring.
How long has the physician performed the procedure?
Our surgeons have between 20-30 years of experience doing these procedures. On larger surgical cases, our surgeons will often work together to get the patient the best possible outcome. Each is board-certified by the American Board of Plastic Surgery.
Who will administer and monitor the sedation?
At the Avalon Clinic for Cosmetic Surgery, you will be closely monitored by a Certified Registered Nurse Anesthetist (CRNAs).
CRNAs are anesthesia specialists and their main role is to monitor and administer the anesthesia medications during the duration of your entire procedure.
CRNAs are board certified and mandatory continuing education is required for re-certification every two years.
How long will the procedure last?
The actual surgery time varies on the type of procedure being performed.
Breast Augmentation (Saline or Silicone implants) is generally less than 1 hour.
Tummy Tuck on average takes less than 3 ½ hours.
Breast Lift takes less than 2 ½ hours
Breast Reduction less than 3 hours
Rhinoplasty (surgery of the nose) less than 2 hours
Liposuctions (length of time varies depending on what areas are treated at the same time) average surgery time ranges between 45 minutes to 3 hours.
*For safety reasons, the Avalon Clinic for Cosmetic Surgery has a standard requirement for the length of operating time. Surgeries generally do not exceed 5 hours.
In some cases, some surgeries may not be performed together for safety or other reasons and in cases such as these; the procedures are done staged (at a later time, usually 3-6 months later)
How long with the sedation effects last?
This will also vary from patient to patient. Some effects of the medications can last up to 8 hours after the surgery.
At the Avalon Clinic for Cosmetic Surgery, we require that anyone having surgery has a "Support Person" to be with them at all times for at minimum, the first 24 hours following any procedure performed at our clinic.
How long will it take to recover in the facility before discharge?
Once the surgery is completed, the patient is transported to the recovery room where they normally will spend approximately 40 minutes to 1 hour before going home. During this time, the patient is monitored and a nurse checks vitals. The support person is asked to be present to go over post-op medication instructions and what to expect in the recovery process.
Where will the surgery take place?
At the Avalon Clinic for Cosmetic Surgery, we have 2 certified and QUAD A accredited operating rooms on site. These operating rooms are subject to regular frequent inspections are required to follow OSHA regulations and standards.
What are the emergency plans?
Robert J. Kenevan, medical director and surgeon for the Avalon Clinic of Cosmetic Surgery, developed Kode Team, a pharmaceutical emergency / safety system to be used in hospitals and out-patient surgical facilities such as ours. The entire staff at the Avalon Clinic is certified in CPR, the Operating room staff is ACLS (Advanced Cardiac Life Support) certified. In addition to that, the medical staff trains 3 times per year with local paramedics. We know that emergencies can and do sometimes occur. We take patient safety seriously and understand the value of emergency training.
To which hospital would I be taken?
If an emergency situation arises that cannot be handled at the clinic, patients are transported to St. Clare hospital by ambulance. The hospital is less than 2 miles from our clinic.
Will post-operative medicine be necessary?
Most patients experience discomfort after surgery. Pain medication is prescribed after surgery and with some procedures such as a tummy tuck, a comfort pump is also given. These will help minimize the discomfort during your recovery process.
Will there be noticeable scarring?
All surgery involves some scarring, and because each patient is unique you need to discuss your questions about scarring with your surgeon during your consultation.
What realistic result can I expect?

Some people think that just because they are paying for something that they can have it done. We do turn away, approximately three out of twenty patients, who are not a candidate due to excessive weight, smoking (for certain procedures), a history of blood clots, or certain medications such as coumadin (a blood thinner). We want you to have an accurate expectation of exactly what plastic surgery can achieve for you.
Will the physician perform the procedure from the first cut to the last stitch?
Yes. The doctor will remain with you throughout your entire procedure.
What are the risks associated with this procedure?
All surgeries carry some degree of risk. During your consultation you will be fully informed, and your surgeon will discuss the specific risks and complications associated with your procedure. Be sure to inform your doctor of any medical conditions you have, if you smoke, if you are on any medications, and if you have had any previous surgery. We want to get a complete evaluation of your medical history and/or existing medical conditions so we can provide you with maximal safety. In some situations we will ask you for a medical clearance from your physician in order to proceed with surgery. Without this clearance from your physician, surgery will not be completed at The Avalon Clinic. Our surgeons also meet three times a month to discuss certain patient situations as a group.
What are the warning signs or concerns to watch for?
We provide training for your caregiver to make sure they are able to recognize any warning signs, such as illness or allergic reaction. They are given information prior to your surgery and again in the recovery room so that they are fully prepared to take care of you. We have a nurse and surgeon on call and available 24 hours a day to answer any questions you might have during the healing process.

The article talks about the national trend of these “mommy makeovers” which has been on the rise over the last several years.
What is a “mommy makeover?”
A mommy makeover is aimed at women of child bearing age and focuses on the two body parts that are almost always affected by pregnancy and aging, the breasts and abdomen. Post-baby bodies is what we call the “mommy makeover” and it often includes a breast augmentation (with or without a lift) and a tummy tuck.

This article pointed out the high number of women and “moms” who seek to reverse the impact of pregnancy and aging on their bodies. Pregnancy affects each woman differently, with age and genetics playing a role in how the body recovers or ages.
With cosmetic surgery becoming a main stream phenomenon, women no longer have to accept their deflated or sagging breasts and lose or stretched skin from having their children like the generations of women before.
One mother, who during her last pregnancy gained 55 pounds, was embarrassed about how she looked after the birth of her second child. She had stretched, loose skin on her stomach after the birth of her second child and even though she did a hundred sit-ups in the morning and 100 sit-ups each night, nothing would make her abdomen flat. Add the lost fullness and deflated appearance of her post baby breasts and she felt very frustrated.
After 3 years of exercising without the results she wanted, she sought out to have her own “mommy makeover” of her post baby body.

Despite that this recent article claims that there are no “intrinsic abnormality to the breasts or abdomen” the problem is that we did a survey of our patients and found that the majority of women seeking cosmetic surgery after pregnancy are in fact…

According to the American Society of Plastic Surgeons (ASPS) doctors nationwide performed over 325,000 “mommy makeover procedures” in 2006. This is up 11% from 2005. The majority of women do not view cosmetic surgery as the easy way out of exercising or going to the gym. In fact, many women have already spent months in the gym with a trainer before they even seek out the opinion of a board certified cosmetic surgeon.
There are some things going to the gym just can’t fix.

Attention! Information You Need To Know When Planning Your Cosmetic Surgery Procedure

Have you made the decision to have a cosmetic surgery procedure?
Then now it is time to learn how to prepare yourself, your body and your support system about what to expect throughout the process.
Very often, people can become overwhelmed by major changes or tasks. The more prepared you are for your surgery, the more relaxed and reassured you and your support system will feel.
Here are four things you can do to prepare for your procedure.
1. Define your goals.
*Write down a list of goals you'd like fulfilled through your cosmetic surgery procedure, including emotional as well as the physical.
2. Get physically prepared for your surgery.
*Ask how you can maximize the results of your procedure through healthy lifestyle choices like diet, exercise, and sun protection.
*You may have to take specific supplements or medication prior to your surgery.
*If you are a smoker, it is likely that you will have to quit two weeks prior to your cosmetic surgery procedure.
*Remember, surgery forces the body to go through a great deal of healing, so the better you take care of yourself before hand, the easier it will be to heal.
3. Schedule your surgery and plan for recovery.
*If you think you may want to have additional cosmetic surgery in the future, consider having two or more procedures performed together to save time and costs.
*Pay attention to the description of the procedure. The more people know about what is happening to their body, the less anxiety they have both before their surgery and afterwards.
*Make sure you understand just how long you will need to heal from your cosmetic surgery procedure.
*In almost all cases, patients are not able to drive or walk unassisted after cosmetic surgery, you will most likely need someone to drive you to your surgery, drive you home and remain with you for the first 24-48 hours. It might be a day or two before you may begin to take care of yourself.
* Schedule ample recovery time away from work and plan on following the doctor's recovery protocol very closely. Trying to do too much too soon following surgery can jeopardize your health and your results.
*Make sure you have help during your recovery time - if you have children, make arrangements for some support in the home so you can truly relax during your recovery period.
4. Schedule your follow up visits.
*With almost all types of cosmetic surgery, you will need to return to your surgeon one or more times afterwards. Your surgeon will assess how you are healing, inspect your incisions, and remove stitches or bandages.
*Your surgeon should inform you of what to expect next in the healing process and when to come return.
*Many times, your follow up visits are scheduled prior to surgery.
*It is advised to keep all of your paperwork and information stored in one place.
Remember, it is important to verify that your cosmetic surgeon is board certified by the American Board of Plastic Surgery and operates only in a fully accredited and certified operating room.

Does it work?
Studies suggest that patients are generally pleased with the outcome and report improvement in self-esteem, social confidence and quality of life.
However, cosmetic surgery may not be appropriate for every person or every problem. If you are considering cosmetic surgery, take some time to contemplate these questions:
Why am I considering cosmetic surgery?
The healthiest reason to have cosmetic surgery is to improve your self-image. Its purpose is to enhance your feeling of well-being and to foster emotional health.
Am I having cosmetic surgery to please others or myself?
Cosmetic surgery cannot stop your boyfriend from leaving you. It cannot bring you new friends. It cannot get you a better job. You'll likely be disappointed if you are having cosmetic surgery to please someone else.
Are my expectations realistic?
Cosmetic surgery can dramatically improve your appearance, but it has its limitations. It is unwise to expect that cosmetic surgery can create the face of a celebrity or restore lost youth.
Am I emotionally prepared for cosmetic surgery?
There are certain circumstances under which cosmetic surgery may be inappropriate. These include a crisis or an emotional upheaval such as divorce, death of a spouse or loss of a job. Also, surgeons are reluctant to consider cosmetic surgery on patients who are depressed or have significant mental illness.
Is now the best time for cosmetic surgery?
Plan your surgery when you are relaxed and can afford the time to convalesce and heal afterward.
Am I prepared to handle unexpected results?
Unexpected results occur rarely, but when they do occur, they create emotional distress for both the patient and the surgeon. You must be prepared for the worst-case scenario and realize that it will require time, patience and a mutual trust between you and your surgeon to work toward improvement.
Do I have a support network?
It's important to have someone to support you physically and emotionally during your recovery. Beware of negative comments from friends or family members who may have issues with your decision to change your appearance.
Have I considered and accepted the risks?
There is risk involved with any surgery. In addition to the general risks related to anesthesia, blood loss and infection, specific risks for cosmetic surgery include skin loss, asymmetry, slow healing, numbness and minor dimples and puckers
Can I afford cosmetic surgery?
Cosmetic surgery is usually not covered by health insurance. If you can't afford the cost, the added financial stress could actually make you feel worse about yourself.

Implant Sizing...At Home, It's Easy!
If so, then you have probably wondered what size would be right for you.
The following information is designed to help you discover what size you would like to be in the privacy of your own home! By following these simple steps you have the freedom to try on many different sizes and wear them around for as long as you would like.
This experiment will give you more freedom to try out different volumes in the privacy of your own home.For many women, cc’s does not mean anything to them and they tend to speak in cup sizes, not volumes. Breast implants that are used in breast augmentations are filled according to the volume of the implant, which is done cc’s.
We know that for most women this will not give them any idea of what cup size that would be for them. That is why by following these instructions you can get an idea of the volume range that works best for you.
So, in order to try this at home for yourself here is what you will need:
•Qt. size baggies (preferably that zip closed)
•Measuring cup with Milliliters (Milliliters is the same as cc’s)
•Sharpie pen
•Sports bra
You can use the bathroom or kitchen, just find a place that has a faucet and counter top to set your supplies out.The first thing you want to do is put on your sports bra. Get near a faucet that allows you some counter space to lay your supplies on.
Depending on your current anatomy and what size you think you would like to be you want to fill your measuring cup with WARM water using the milliliters side. Milliliters are the same as cc’s, so for example, a 300 cc implant is the same as a 300 ml of water.Pick a starting volume and fill baggie with warm water.
When done, try to get out as much air as possible. Fill 2 baggies (one for each side) and then place them inside your bra. It might be helpful to keep track of the volumes you are using and what you like.
It’s as simple as that!
Have fun experimenting with different sizes and make sure that you try them on with some shirts also so that you get a better idea of what the changes would mean to you!

All About Botox
What is Botox?
Botox Cosmetic is a purified protein (Botulinum Toxin Type A) produced by the bacterium Clostridium botulinum, that is injected directly into the tiny facial muscles that contract and cause wrinkles. The injected toxin will then weaken or paralyze the muscles, causing the overlying skin to smooth out.
Is it harmful?
No, it is not harmful. Acceptable safety in long-term treatment has been well established.

How long does the Botox treatment take?
